HAZMATEAM Inc. has several ways in which you can register for our courses.  Please choose the most convenient method.  We suggest you review our Course Policies before registering for a course.

Course Policies

Confirmation:

Upon receiving your registration, HAZMATEAM will mail your class confirmation, course information and directions.

Cancellations:

If you are unable to attend your scheduled workshop, HAZMATEAM highly recommends that you substitute if possible.

We require 10 working days notice for cancellations. A 33% cancellation fee will be applied for all cancellations received with fewer than 10 days notice.

If no cancellation is received, a 50% fee will be applied.

Limitations:

HAZMATEAM reserves the right to cancel any scheduled presentation due to: weather conditions, lack of sufficient class enrollment, or other circumstances beyond its control. In the event of inclement weather, please check our Storm Center for delays or cancellations.

HAZMATEAM will promptly refund all monies received for the course, should cancellation be necessary.

Fill out the registration form.  Print this page from your browser and fax it to (603) 882-6512. Follow up with a check or purchase order made out to HAZMATEAM, Inc.

Mail
Fill out the registration form. Print this page from your browser and mail it to HAZMATEAM, Inc., 12 Kimball Hill Road, Hudson, NH 03051-3915.

Internet
Fill out the registration form. Verify that all of the information you entered is correct, then click on the "Register" button at the bottom of the form. 
 

Methods of Payment

By check:

Made out to HAZMATEAM, Inc. and mailed to PO Box 751, Hampstead, NH 03841-0751, or brought to class with you.  Please include name of person taking class and the course name.
 

By credit card:

We accept Visa, MasterCard and American Express.  Call Karen at 603-882-1112 with card number.

By invoice:

Our terms are Net 10.  As long as your company is not new to us, you can be invoiced after the completion of a class.  Please provide us with a Purchase Order Number and complete billing details if your Accounts Payable Department is located off-site.
